Bring all related medical records and original diagnostic films (X-rays, MRI, etc.)
Bring a valid insurance card and driver’s license.
Bring a list of your current medications.
Bring the referral from your primary care physician, if required.

Your doctor will perform a thorough examination. We will provide a hospital gown to change into as needed. However, most patients are more comfortable in their own clothing, and we might suggest you bring in comfortable gym attire (i.e. shorts & t-shirt for men, shorts & sports bra for women).
